Affordable Dental Care Membership*

No Insurance, No Problem!

Our membership plan offers significant value for just $1 a day, totaling $365 for the first year. To enroll, a $150 down payment is required, followed by convenient monthly payments of $19.50.

Upon joining, members will immediately receive 20% off all restorative and oral hygiene procedures.

*After the 1st year, the membership plan will auto renew until cancelled by the patient. We will require a credit card or debit card to be placed on file for monthly charges, we will NOT invoice. Patients will be able to choose what day of the month to have the funds deducted from the card on file. A 30-day notice will be required to cancel the plan after the 1st year to allow time to stop the autopay feature in our software. This membership CAN NOT be combined with any dental insurance used in our office or additional promotions, or the New Patient Special. If a patient already purchases the New Patient Special, an additional 20% off will NOT be applied to the $89 (already discounted) price. If patients have an HMO or discount insurance plan, we can offer the membership plan, but patients need to understand that they will not be able to get reimbursed by their plan due to the ADA code used for billing is a Cruzin' Dental exclusive and is not nationally recognized, which is how carriers reimburse their subscribers. After one missed monthly payment, we will try to run card on file 3 days after missed payment. If unsuccessful, we will contact the patient and leave a message. IF unresolved after one week we will send an invoice. After 2nd missed payment, repeat steps. After the week of the 2nd attempt the plan will be cancelled and patient will have to purchase and start membership over.
